SUMMARY:No Carve Out for Clinical Decisions: Best interests and clinical judgement after Townsend
DESCRIPTION:The Court of Appeal decision in Townsend v Epsom and St Helier University Hospitals NHS Trust has raised important questions about clinical decision-making and best interests under the Mental Capacity Act 2005. \nThe case concerned Mr Barnor\, a 68-year-old man with irreversible brain damage who required dialysis. The court ruled that decisions about the care and treatment of adults who lack capacity must be made in their best interests\, with “no carve out” for clinical decisions. This has prompted debate about the distinction between clinical judgement and best interests decision-making. \nWith the Supreme Court hearing scheduled for January 2027\, join our panel of legal and clinical experts to explore the judgment and its implications for everyday practice. \nWhy attend? \nThe webinar will explore the legal and clinical questions arising from the judgment\, including how healthcare professionals should approach best interests decisions and disagreements about treatment while awaiting the Supreme Court’s ruling. \nSpeakers \n\nProf Emma Cave\, Professor of Healthcare Law\, Durham University\nDr Alex Cisneros\, Barrister\, 39 Essex Chambers and Visiting Lecturer\, King’s College London\nDr Jan Dudley\, Consultant Paediatric Nephrologist\, Bristol Royal Hospital for Children\nDr Ben Thomas\, Consultant Nephrologist\, Betsi Cadwaladr University Health Board

SUMMARY:ISN-UKKA Webinar: Dialysis or conservative kidney management for kidney failure – the first results of the Prepare for Kidney Care randomized controlled trial
DESCRIPTION:Prepare for Kidney Care is the first randomized controlled trial comparing preparation for dialysis with conservative kidney management in older people approaching kidney failure. \nWhile a combination of quality and length of life formed the primary outcome\, many other outcomes relevant for shared informed decision making have been studied. \nPrepare for Kidney Care was conducted across 31 kidney units in the UK\, recruiting 445 participants who were then followed for an average of 4.5 years. This webinar will begin by setting out what is already known about this topic\, before moving on to explain the design and present the results and conclusions. An expert patient and independent expert health professional will then reflect on the findings. Finally\, there will be an opportunity for webinar attendees to put questions to the panel. \nLearning objectives: \n\nTo understand what is currently known about the effectiveness of dialysis and conservative kidney management in older people living with kidney failure and multimorbidity or frailty.\nTo learn whether/ how the relative effectiveness of the treatments changes when a randomised controlled trial approach is used to ensure a fair comparison between the two treatments.\nTo understand when\, and in whom\, the findings can be used in shared informed decision making\, and when\, and in whom\, they might need to be used more cautiously.\n\nFurther reading: \n\nThe Prepare for Kidney Care Study: prepare for renal dialysis versus responsive management in advanced chronic kidney disease\nPreparing for responsive management versus preparing for renal dialysis in multimorbid older people with advanced chronic kidney disease (Prepare for Kidney Care): Study protocol for a randomised controlled trial\n\nModerators: Meg Jardine (Australia) & Jos Darling (UK) \nSpeakers: \n\nBarnaby Hole (UK)\nJo Worthington (UK)\nFergus Caskey (UK)\nFiona Loud (UK)\nJane Schell (USA)
