Knowledge Hub

The Transform AKC Knowledge Hub brings together learning from across the programme to support quality improvement in advanced kidney care.

Explore the changes being tested by participating kidney teams, learn from the experiences of people receiving advanced kidney care and access resources, webinars and shared learning to inform quality improvement in your own service.

The pilot centres

Five kidney centres are testing improvements in different areas of advanced kidney care. This allows the programme to explore different parts of the AKC pathway while sharing learning between participating teams.

Patient education

Improving patient education and engagement, including reviewing information and resources, developing more tailored approaches and reintroducing face-to-face education sessions.

Frailty and shared decision-making

Using frailty assessment to support more personalised care and shared decision-making for people approaching kidney failure.

Symptom management

Improving how symptoms are identified, monitored and managed, including testing symptom assessment tools and strengthening pathways for responding to patients’ symptoms.

Pre-emptive transplantation

Improving the pathway towards pre-emptive transplantation, including better recording of transplant status and identifying delays and opportunities for improvement across assessment and listing.

Health literacy

Identifying health-literacy needs within the AKC population and exploring how this information can be used to make patient information, education and communication more accessible.

Transform AKC Engagement Heat Map

Sharing learning

Transform AKC is intended to generate learning that can be used beyond the five pilot centres.

Healthcare professionals from across the UK have contributed to the programme through webinars, workshops and shared learning events, while the pilot teams come together to share progress, challenges and approaches.
