What is integrated CKD?

Integrated CKD care brings together primary care and specialist nephrology services to improve the identification, management and outcomes of people with chronic kidney disease. Although models vary across the UK, most are built around six core pillars: early identification, risk stratification, medicines optimisation, integrated multidisciplinary care, workforce development, and health equity.

  • Early identification and accurate coding: finding people with CKD earlier and ensuring records support proactive management.
  • Risk stratification: using and embedding tools such as the kidney failure risk equation (KFRE) to match care and specialist input to patient risk.
  • Medicines optimisation: ensuring evidence-based therapies and blood pressure control are used effectively to slow CKD progression and lower cardiovascular risk.
  • Integrated multidisciplinary care: bringing primary care, nephrology, pharmacy and other professionals together around shared pathways.
  • Workforce development: building confidence, skills, and capability across the workforce.
  • Equity and population health: reducing inequalities and ensuring underserved populations benefit from CKD improvement programmes.

The Integrated CKD Knowledge Hub

The Integrated CKD Knowledge Hub brings together the resources, data, tools and real-world examples needed to support integrated CKD care.

The hub includes:

  • Education and training
  • National data and dashboards
  • Examples of integrated CKD programmes from across the UK
  • Links to relevant guidelines, tools, and patient resources

By bringing together knowledge and resources that are often spread across multiple organisations and regions, the hub makes it easier for teams to learn from existing work, share best practice, and accelerate improvement.
